I purchased the Provent and GU 3.0 kit from Western Filters. All arrived as advertised BUT when I tried to fit the return hose it turns out it is 16 mm, not the 12 mm of the input hose. Nothing in the kit says anything about size differences.
So if you plan on fitting a Provent to a 2006 GU 3.0 Di, plan on getting some 16 mm hose and a 12mm to 16 mm coupling if you go that route. I just fitted the 25 mm hose about 50mm long to each Provent connection, then fitted a copper sleeve in both hoses (to prevent crushing) and clamped them to the 25 mm hose past the end of the Provent . Both hoses are pushed into the Provent connections.
Works great and is tidy.

I did replace the bolts with correct length once I had it all sorted.

Yes, I found the provent outlets a strange size too. I used black plastic watering system fittings, as they were easily able to adapt to the provent. Don't do this! In the middle of replacing it, as the pressure doesn't agree with the fittings, and slowly leaks. Dust then sticks to the outside of the fittings.
(and I've also used Western Filters. Rating: I'd definitely buy from them again without too much thought.)

Chase up some Xc-Xe falcon heater hose from rare spares. Fit's snug both ends and no reducers / adapters.

Yup, Western Filters did exactly as promised, so no issues there. I did find a 25mm threaded connector with a 16mm barb, so now I can redo the connections using Westerns instructions--a long road to here

The CRD's hose size is the same at the motor and at the intake I believe Eric. The Di's are 1/2inch (from the motor) and 5/8inch( at the air intake end) in the old scale.
To the OP its a nice and snug fit in there hey! Close to the ABS lines...

Definitely not mate. Reduction is needed, I'm not sure what size exactly though.

Hmm ok. They must be the same as the DI then.

On my Di I just flipped existing OEM hose 180° (to face backwards) and used 16 mm (5/8') end to connect via 5/8 hose joiner to another hose to the Provent.
For the record, the rocker cover fitting is 12 mm and intake pipe end is 16 mm.

The Provent is CLOSE to the ABS, so I have sleeved the closest ABS line to ensure no rubbing.
Definetly 12mm at the tappet cover, and 16 mm at the inlet on my DI
