My wife suffers from Sciatica to the extent that she has difficulty walking at times. She went through the medication, physio, and cortisone injection path with no positive result.
Several years ago a specialist anesthetist recommended a caudal injection, as surgery was the next resort she decided to try the injection first. It's done while in a CT scanner and it had pretty much immediate pain relief.
Since then she has had caudal injections every few years, sometimes they take three weeks to show improvement other times its nearly instant.
Not sure if this works for everyone but for my wife it was a life saver, might be worth checking out.
