Have you got movement in the steering box(bolted onto the chassis under right side mud guard)? With the engine off but key in ignition to unlock the steering lock, I was able to move the steering wheel back and forth and the steering box would make a clunking noise. Fixed this by adjusting the steering box. There is an adjusting screw ontop of the box, just loosen the large jamnut and adjust the screw in the center in 1/8 of a turn increments. I don't know if this wil fix your problem, but I have heard of people fixing their death wobbles by adjusting the steering box.
When you do adjust it, test the steering from lock to lock to ensure that it moves without binding. I'm not 100% sure on what happens within the box when you adjust it, but I'm thinking that adjusting it too much would reduce the acceptable amount of backlash between the gears, causing binding.
