I used this as the analogy to a recent fire on site in a heat exchanger where hot LPG is heated to remove ammonia. The reasons for the event are wide and humorous, but none better than the statement by the reliability engineer who claimed the issue related to the heat exchanger being incorrectly designed - yeah right, its been in service for 20 years and never caught fire - maybe look t your process operation???
