TB45 Jerking on low revs

Hi guys,

I have an issue that I hope you can help me with.
When I drive my 4.5 patrol in town it feels like its jerking the whole time.
Like the revs will be at 1500rpm and then it will drop to 1400rpm and then back up.

I changed the plugs, fuel filter, air filter and cleaned the maf sensor with sensor cleaner but it still does it?

What else can I check?
Its got 220000km on.

Check all the small vacuum lines for cracks and make sure all are fitted
sounds like a vacuum or air leak of some description

Change the plug leads and if it still continues have a look inside the distributor cap..
